Family Welfare Coach (Building Successful Families)
Part-time: 29.6 hours per week
Salary: £25,166 per annum pro rata (dependent on experience & qualifications)
2-year initial fixed term (and 1-yr extension subject to funding)
KEY TERMS & CONDITIONS
25 days holiday & bank holidays pro rata and 8% non-contributory pension
1-day birthday leave

PURPOSE OF JOB
The Welfare Coach forms a key part of SOAR’s Social Prescribing Service. Working closely with the Sheffield Safeguarding Hub to provide intensive support for families who have a range of complex needs. The role will:
Provide tailored emotional & practical support to help individuals experiencing financial and social hardship, achieve a ‘simpler life’
Complete an early help assessment of the family’s needs and wishes that identifies ‘what matters to them’ and enables the right level of support to be put in place, that prevents any issues to escalate.
Co-ordinate, simplify and broker access to a range of health and social interventions based on the needs and wishes of the individual, including liaising with a range of agencies (schools, housing, Family Intervention Service - FIS)
Empower the individual to express their views, wishes and concerns
Connect individuals to local and city-wide services
Develop close working partnerships with health, social care, housing and voluntary, community or social enterprise (VCSE) organizations.
