Adult social care is here to support people who need extra help with everyday life because of an illness, disability or change in circumstances.
To decide what kind of support you might be able to get, we’ll offer you a Care Act assessment.
Care Act assessments
This is a conversation about what’s working well, what’s difficult and what kind of help might make things easier. You’re likely to be eligible for ongoing support if:
- you have needs linked to a physical or mental condition
- you’re finding it hard to manage two or more everyday tasks
- this is affecting your wellbeing
Following the assessment, we will decide whether you are able to receive support from us. This is known as ‘eligibility’.

If you are eligible for support
We’ll work with you to create a care and support plan. This could include care services, help at home, equipment or community support. We’ll also:
- carry out a financial assessment to see if you need to pay anything
- offer you a personal budget so you have choice over your support
- help arrange services if you want us to
If you are not eligible for support
You won’t be left without help. We’ll still offer advice, signpost you to other services, and help you find support in your community.
