Request an occupational therapy assessment

If you or someone you care for is struggling with day-to-day tasks at home, our occupational therapy team can make daily activities easier, reduce risks, and support independence.

Occupational therapy can help you live your best life by enabling you to do the things you want and need to do at home.

We can support you to overcome challenges in everyday tasks like getting dressed, going out, and doing the dishes. We can give advice, provide equipment and recommend home adaptations.

We’ll work with you to create a plan that fits your life, not just your diagnosis. We can offer solutions over the phone, at our clinic or through home visits.

Who can get support

You can use this service if:

  • your ordinary residence is Sheffield
  • you are aged over 18
  • you are finding everyday tasks difficult
  • you are at risk of injury, hospital or residential care admission

For sudden declines in function, please refer through Health, Single Point of Access (SPA) on 0114 226 6500.

When occupational therapy may not be right

Our team may not be the best service if:

  • you already receive continuing healthcare funding
  • you're getting support from another occupational therapy team (for example, in hospital)
  • another service is better suited to your needs (see below)

Speak to your GP or SPA if you need:

  • walking aids or a specialist bed
  • support with pain management or pressure care
  • rehabilitation or therapy to improve your mobility or daily skills
  • urgent equipment to avoid a hospital stay

Request an assessment with occupational therapy

What happens next

Once we have your referral, we will contact you by telephone to talk through your needs. We might:

  • give advice or suggestions straight away
  • invite you to our independent living clinic
  • refer you to another service if more appropriate
  • add you to our waiting list for a home assessment

If you're added to the list, we'll let you know about waiting times and give you advice on how to wait well and stay safe.

While you're waiting, you may find it helpful to explore other support available in your local area.

The Sheffield Directory includes information about:

  • local voluntary and community groups
  • support services across the city
  • equipment you can buy yourself
