Last night (Monday 4 November), young residents of Sheffield who go above and beyond to help their schools, communities and good causes were thanked for everything they do.
A new Independent Living Clinic has just launched at The Manor Neighbourhood Centre - another step towards making it easier for people with a long-term health condition to access equipment to support them to stay well and independent at home.
More people in Sheffield who need support are living independently and well at home because of work between health and care services to prevent readmission to hospital.
