Find the right support to help you live independently after leaving hospital, prison or care.

Support is available if you're in hospital and don’t have a safe or suitable place to go when you're discharged. 

Let your hospital care team know as early as possible. They can use our duty to refer application to refer you to a Housing Support Discharge Worker. They will help improve your current housing or look for other options before you leave hospital.

Housing support is available if you don’t have a place to stay when you leave prison.

Tell your Probation or Prison Officer as early as possible. They can use our duty to refer application to get you help in improving your housing or in finding a new place to live before your release.

Care leavers

There is housing support and guidance for care leavers up to age 25.

The Leaving Care Service can help find suitable accommodation or apply for social housing.

Your Personal Advisor will support you in setting up your home and visit you at your accommodation, to check that you are managing.
