Registering a stillbirth
We understand that it can be very daunting to have to face registering a stillbirth. These pages are designed to guide you but if you would rather speak to someone personally, please contact the appropriate Register Office.
If the baby was born in Sheffield, please telephone the Register Office on 0114 203 9427 between 8.30 am and 4.00 pm Monday to Friday, or call into the office (open 9.00 am to 4.00 pm Monday to Friday), to make an appointment. Location of Sheffield Register Office
If the baby was not born in Sheffield, either contact the Register Office for the district where the birth took place or telephone, or call into, this office and ask about registering by declaration.

Who can register?
Either parent provided they were married to each other at the time of the still-birth. If the parents were not married and they wish to include father's details, both parents should be present, otherwise the mother can attend alone.
What will I be asked?
In addition to the medical certificate issued by the midwife or doctor, you will need to provide the following information:
- The date and place of the still-birth
- Any names the parent(s) wish to give the baby
- Mother's (and father's if applicable) full name, place of birth, occupation and usual address
- Mother's maiden name (if applicable)
What will the registration cost and what documents will I receive?
- Registration is free of charge. We will give you a certificate of registration of a still birth which shows the child's full name (if given) and date of birth. A stillbirth certificate, which is a certified copy of all the details held in the register, is also available for £3.50 per copy at the time of registration. If you need certified copies at a later date please contact the appropriate Register Office.
- A certificate for burial or cremation - this is required by the funeral director
