Your Care Record Guidance: Appendix 3: Roles and responsibilities
1. To issue and contribute to an existing folder:
a. Sheffield City Council Neighbourhoods and Community Care (Adult Social Care)
- Older People’s Assessment and Care Management Service
- Physical Disability and Sensory Impairment Service (including Acquired Brain Injury social workers, Transitions Team, and Sensory Impairment team where ongoing support is offered.)
- Joint Learning Disabilities Service
b. Sheffield Primary Care Trust
- Community Nurses
- Community Matrons
- Intermediate Care
- Therapists
- Specialist Nurses
- Specialist services e.g. Acquired Brain Injury
c. Sheffield Care Trust
- Joint Learning Disabilities Service
2. To contribute to existing folder:
a. Sheffield Teaching Hospitals Foundation Trust staff (when care record taken into hospital)
b. Sheffield Care Trust
- Older Adult Mental Health
c. Sheffield City Council Neighbourhoods and Community Care
- Physical Disability and Sensory Impairment Service - Forge Centre social workers. Sensory Impairment team where ongoing support is not offered, Equipment and Adaptation services, Community Support Services, Adult Family Placement Services
- Older People’s Assessment and Care Management Service assessors in Accident and Emergency N.G.H. and Intermediate Care Teams out of hours social workers
- Neighbourhoods Housing Solutions

- GPs
- Pharmacists
- Podiatrists
