Your Care Record Guidance: Appendix 3: Roles and responsibilities

1. To issue and contribute to an existing folder:

 
a. Sheffield City Council Neighbourhoods and Community Care (Adult Social Care)
  • Older People’s Assessment and Care Management Service
  • Physical Disability and Sensory Impairment Service (including Acquired Brain Injury social workers, Transitions Team, and Sensory Impairment team where ongoing support is offered.)
  • Joint Learning Disabilities Service 
 
b. Sheffield Primary Care Trust
  • Community Nurses
  • Community Matrons
  • Intermediate Care
  • Therapists
  • Specialist Nurses
  • Specialist services e.g. Acquired Brain Injury
 
c. Sheffield Care Trust
  • Joint Learning Disabilities Service 
 

2. To contribute to existing folder:

 
a. Sheffield Teaching Hospitals Foundation Trust staff (when care record taken into hospital)
 
b. Sheffield Care Trust
  • Older Adult Mental Health
 
c. Sheffield City Council Neighbourhoods and Community Care
  • Physical Disability and Sensory Impairment Service - Forge Centre social workers. Sensory Impairment team where ongoing support is not offered, Equipment and Adaptation services, Community Support Services, Adult Family Placement Services
  • Older People’s Assessment and Care Management Service assessors in Accident and Emergency N.G.H. and Intermediate Care Teams out of hours social workers
  • Neighbourhoods Housing Solutions

  • GPs
  • Pharmacists
  • Podiatrists
