Who can look at the Your Care Record folder?

Unless it's an emergency, or a community nurse or therapist needs to look at their notes, people can only look inside your folder if you say they can.
 

Should I ask my GP (doctor) or Emergency Care Practitioner to write in my folder?

Yes, if you want to. We expect GPs and Emergency Care Practitioners to write in the Record of visits section of your folder.

Can I use my folder to communicate with my Single Assessment Process (SAP) Coordinator/key contact person?

No - we strongly advise you to continue to use existing methods of communication, (for example, telephone) to make sure an appropriate level of care is maintained.
 

What happens in an emergency?

In an emergency, people like ambulance staff or paramedics might need to look in your folder without your permission. They'll be looking for information about your needs and who else is involved in your care.
 

What if I don't want a folder?

You don't have to have one - it's your choice.
 

What happens if my folder gets full?

Practitioners involved in your care will help you to manage the information in your folder. If your folder gets full, you can take some of the older information out. You can keep the information somewhere else, or destroy it. We recommend that if you want to destroy it, you shred it. If you prefer, you can give the information to your SAP Coordinator or other practitioner involved in your care to destroy.
 

What if I lose my folder?

You should let your Single Assessment Process (SAP) Coordinator (key contact person) know. Copies of most of the information in your folder will be held with the organisations involved in your care, so you should be able to get a new folder if you'd like one.
 

What if I take my folder to a hospital appointment and leave it there by mistake?

If we find your folder, we'll make sure you get it back. If we can't find your folder, we'll contact your Single Assessment Process (SAP) Coordinator (key contact person) to let them know. Copies of most of the information in your folder will be held with the organisations involved in your care, so you should be able to get a new folder if you'd like one.
 

If I move out of Sheffield, can I take my folder with me?

Yes, you can. However Primary Care Trust (PCT) staff will need to take out their notes and replace them with a discharge summary which will explain about your needs and your care.
 

Is the folder available in other languages?

No, the folder is only available in English. We will work with you to help you understand the content of your folder and will use interpreters where necessary.
