Developing Your Care Record
Aims
Your Care Record aims to:
- be a means of involving people in their care by sharing information with them and their carers, as part of the person-centred approach of single assessment
- improve information flow and communication between different groups involved in the person’s care, by allowing assessment records and other relevant documents to be held centrally in a recognisable, standardised format
Organisations involved
Your Care Record forms part of the city-wide development work for the Single Assessment Process (SAP).
The following organisations were involved in developing Your Care Record:
- Sheffield City Council
- Sheffield Primary Care Trust
- Sheffield Teaching Hospitals NHS Foundation Trust
- Sheffield Care Trust.
These organisations are all members of the Sheffield First for Health and Well-being Partnership.
The pilot record
A Personal Assessment and Care Record (PACR) was developed in 2006 following consultation with key stakeholders, including people from other communities across the country who already had similar records in place.
The PACR was piloted between November 2006 and February 2007 in the West of Sheffield.
Evaluating the pilot
The pilot was evaluated between February and April 2007. The evaluation was led by a Clinical Audit and Effectiveness Facilitator, Sheffield Primary Care Trust.
We evaluated the pilot using evidence gained from:
- questionnaires to staff, service users, carers and home care providers
- an audit of documentation
- a feedback form included with the pilot folders
- interviews with service users and carers

Implementing Your Care Record
We used the lessons learned and recommendations from the audit to improve the PACR content and supporting guidance. This resulted in a redesign, and a new name – Your Care Record.
Your Care Record was implemented across Sheffield in Spring 2008.
