The Carer's alert card
Do you care for an adult or a child with a disability who couldn't cope without the support you provide? If so, you may be interested in applying for a Carer’s alert card. It’s a new scheme for adults from Sheffield City Council and the Sheffield Carers Centre.
How does the scheme work?
You fill in an application form nominating people who can help the person you care for if you’re involved in an accident.
Return the form to us and we’ll send you an alert card, which has your name and a reference number on the back. We’ll also give you a plastic holder that fits into your purse or wallet so the card can be seen from the outside.
If you’re involved in an accident or incapacitated (for whatever cause) the emergency services will look for this card. They’ll call the 24-hour emergency number on the back of the card, and pass on your name and reference number.
Our City Wide Alarm service will then try to contact the people you nominated to let them know what’s happened. If they can’t reach them they’ll pass on the details of the person you care for to social care services.
The people you nominate can be relatives, friends, neighbours and so on, but they must agree to be nominated.
What happens to my personal information?
All the information that you give us will be kept confidential and won’t be used for any other purpose. The card doesn’t have any personal information on it other than your name.
What does the card cost?
The scheme is free to join.

You’ll need to talk about this with the person you care for and the people you nominate. You must get their permission to give us their personal details. We can't accept your form unless this is done.
