Service Principles

 
  • The Sheffield City Council Physical Disability & Sensory Impairment service (PD/SI) will not  discriminate against any person on the grounds of ethnicity, gender, religion, disability, sexual orientation, marital status or for any other reason.
 
  • The PD/SI service believes disabled people are entitled to live an independent life and seeks to support all service users in achieving this aim where they wish.
 
  • The PD/SI service believes in the social model of disability, and seeks to remove barriers that disable.
 
  • The PD/SI service is working towards increased choice for people who use the service and is seeking to make available a wide range of appropriate and accessible services.
 
  • All staff working within the PD/SI service will respect the views of all service users and their carers, and request that service users respect the staff and service providers who work with them.
 

Customer Service Standards

 
  • If you have a new referral to PD/SI you will be informed within five working days of the priority given to your request and the probable time you will have to wait to receive an assessment.
 
  • Provide an assessment and care management service during office hours Monday – Thursday 8.45 – 5.15 pm and Friday 8.45  - 4.45pm
 
  • Provide an emergency service out of office hours including weekends and bank holidays.
 
  • All meetings will be convened in appropriate venues, which are fully accessible for people with a physical, cognitive or sensory impairment.  They will be convened at appropriate dates and times for the person using the service and their carer if applicable.
 
  • Information will be provided in a timely manner and in appropriate formats or languages.
 
  • We will provide you with information about Direct Payments
 
  • The PD/SI service will assist you to express your views.  If you need an advocate or an interpreter we will try to find one.
 
  • Cultural and spiritual needs will be taken account of during assessment and service provision, and in all activities undertaken by the PD/SI service. 
 
  • The PD/SI service will only share confidential information about you when we have your permission or in exceptional circumstances where there is a legal requirement to protect you or other people from serious harm.
 
  • A statement of need will be agreed between the service user, their carer (if appropriate) and the assessor(s) and decisions on service provision will be based solely on the assessment of the person’s needs within SCC eligibility criteria.  You will be involved in decisions about services.
 
  • You will always be given the details of who to contact and how to contact them.
 
  • Any ongoing services provided by PD/SI will be reviewed at least once a year
 
  • You will be advised of your right to complain if you are not happy about the service you have received
 
  • Aim to deliver items of equipment and small adaptations within 7 days
 
  • The PD/SI service is committed to meeting these standards and will regularly monitor whether or not these standards are being met by a variety of methods.
