Brain Injury Social Work Team

Introduction

 
The Brain Injury Social Work Team is based at Osborn 4 Neuro-rehabilitation Unit, Northern General Hospital.
 
The team consists of
   
  • 2.5 Social workers (one of whom works in hospital settings)
  • 0.5 Senior Practitioner
  • 1 Care Co-ordinator.
 
The Brain Injury Team works as part of a multi-disciplinary team along with colleagues from Health, to provide a personal Social Service to people with an acquired brain injury and their families and carers.
 
The team works with service users while they are in rehabilitation. This can be hospital, community or residential settings.
 
The team currently work with inpatients at Osborn 4, the Head Injury Rehabilitation Unit at Northern General Hospital and into community health rehabilitation settings, and the acute neuro-surgical wards at the Royal Hallamshire Hospital. The team 
 
  • work with clients aged 16 years and over
  • work with service users and their carers in a variety of ways depending on their needs
  • purchase, review and monitor community care and residential care placements including out of city specialist placements
  • offer practical and emotional support to service users and their carers where appropriate
  • offer training and support to other professionals working with the effects of acquired brain injury.
 
Referrals are only accepted via Health / Social Care Professionals, however the team offer support, consultation and guidance to a wide range of professionals and families.
 
Referrals are picked up immediately on admission to Osborn 4 / Head Injury Rehab Centre. All other referrals will be contacted within 10 working days.
 
 
The Brain Injury Team work to Physical Disability and Sensory Impairment service principles and standards, but also has it’s own team standards below
 

Brain Injury Team Standards

 
  1. Referrals are picked up immediately on admission to Osborne 4, Head Injury Rehab Centre
  2. Other referrals, contacted within 10 working days
  3. Everyone will be given advice, information and support appropriate to their needs
  4. Once rehabilitation has ended or care packages have been set up and reviewed the team will transfer responsibility for support to the appropriate community team. The length of time this takes varies from service user to service user.
  5. The team only work with Sheffield clients but Liaise with other local authorities for out of city residents receiving care through the Brain Injury Pathway.

The team work Monday to Friday 8.45am - 5.15pm.
