Disability Access Team

 

You have been referred to the Disability Access Team (social work team) for an assessment of your needs.

 

The Disability Access Team is the first point of contact for adults with Physical and / or Learning Disabilities, 18-64 years old, and their carers/families. The team is responsible for providing assessment to these adults, when they appear to need Community Care services primarily because of their Disability. The team provides a short-term social work service and aims wherever possible to complete our work with you within 6 weeks.

 

The team aims to provide a swift response to requests for assessment. If you appear eligible for an assessment you will be allocated a social worker, who will work in partnership with you and your carer/family (if appropriate) to assess your needs.

 

 

1.    We are busy team and therefore have devised a social worker allocation system, which takes into account the current level of your problems/difficulties. We aim, wherever possible, to assess Disabled Adults within the following timescales :-

 

Priority 3 Cases Waiting Time-   10 weeks   

 

2.    However we are aware that some Disabled Adults may contact the Team at a point of crisis and where a decision is made that a person has to be seen as an emergency we aim to assess their needs within 48 hours

 

3.    Once your assessment is completed and our involvement is complete, your social work file will be closed or transferred to an area social work team, depending on the outcome of your assessment and the support/services provided.

 

At the end of our work with you we will ensure that you have the relevant contact details for the social work team who will assist you if you need social work involvement in the future.
