Community Support Service

The service has three main purposes:
 
  • Promoting social inclusion
  • Providing social, emotional and functional rehabilitation, respite services and working with family carers.
  • The Service seeks to promote a social approach to disability
 
 

Introduction

 
The purpose of the Team is to provide support to customers who have a Physical Disability aged between 18 and 64 who have been assessed as having an eligible need (measured against Fair Access To Care Services, FACS). The Service works around three principles - Respite, Rehabilitation and Social Inclusion. The respite work is predominantly done in group settings whilst rehabilitation and social inclusion work are on a one – to – one basis. Social Inclusion underpins all work done with all service users.
 
The service operates from 7 a.m. to 10 p.m. seven days a week.

Philosophy

 
The Team operates within the Social Model of disability and seeks to increase social inclusion for adults with a physical impairment by:
 
  • Helping people to live more independently
  • Helping people to live safely
  • Helping people to improve the quality of life
  • Provide services that are fair and consistent
  • Developing services which reflect and respond to the expressed needs of the diverse communities of Sheffield, ensuring that individuals are involved in all aspects of their service
 
The main aim is to support Service Users to use mainstream services such as, lifelong learning, employment and leisure increasing independence, choice and inclusion, rather than having ‘separate’ or ‘special’ services.
 
Dependant on an assessment of mobility needs; the service may provide transport to and from activities.
 
The Community Support Service has experience of working with people with brain injuries and is developing this alongside the Sheffield Brain Injury Network.
 
The Community Support Service has worked with, Inclusive Living Sheffield (ILS) to provide training for staff and service users to promote and apply a social approach to disability.
 

Our Team’s Standards

The Team will aim to:
 
  1. Develop a support plan with the individual that will maximise their independence
  2. Review new cases with the individual and their care manager within 3 months
  3. Ensure that the support plan remains appropriate and effective through regular monitoring
  4. Provide support as agreed and when it is expected
 
 

Referral 

Referral to the Community Support Service involves assessment by a social worker and the Disability Resource Allocation Panel (DRAP) makes decisions about providing services based on that assessment.
 

Charges

Charges are made for the Community Support Service, based on the  ‘Contributions for Non-Residential Services Policy’. This involves financial assessment of the individual. Current maximum contribution is £5.50 per day, plus 40p per transport journey.
 

Floating Support Service

 
The Floating Support Service is a new service being provided by the Community Support Service. Floating Support provides support to maintain a tenancy. The service is fully funded for those receiving Housing Benefit or Council Tax Benefit, otherwise a fairer charging assessment is required.
 
This service aims to enable and motivate service users through a regular programme of activities, to undertake tasks for themselves. People can refer themselves for this service. Further information can be downloaded from the Supporting People document below.
