Care Management Teams
The North & South Physical Disability Teams
The purpose of the Teams is to provide Social Work support to customers who have a Physical Disability aged between 18 and 64 who have been assessed as having an eligible need (measured against Fair Access To Care Services, FACS).
The Team operates a Duty system whereby there is a social worker that is office based to respond to urgent calls.
Philosophy
The Team operates within the Social Model of disability and seeks to improve the services provided to adults with a physical disability / impairment by:
- Helping people to live more independently
- Helping people to live safely
- Helping people to improve the quality of life
- Provide services that are fair and consistent
- Developing services which reflect and respond to the expressed needs of the diverse communities of Sheffield .Ensuring that individuals are involved in all aspects of their service
Our Teams Standards
The Team will aim to:
1. Allocate cases on a priority basis:
Priority 1 – within 2 weeksPriority 2 – within 2 – 4 weeksPriority 3 – within 4 – 6 weeks
2. Review new cases within 3 months and thereafter every 12 months
3. Provide a Statement of Need and care Plan to all customers receiving a service

The Team aims to take a Social Model of Disability in its approach. This model views:
Impairment - as the health problem experienced by the individual
Disability - as the limits people face to live normal lives. People are therefore disabled from having equal opportunities
The Team will ensure that individuals are treated with respect, dignity and equality. The Team will take a person centred approach in assisting individuals to achieve independence.
