What is the Physical Disability & Sensory Impairment (PDSI) Service?
The Manager with overall responsibility for the PDSI service is Linda Gregory, Strategy and Operations Manager. The PDSI service us a group of services within the Neighbourhoods and Community Care Directorate of Sheffield City Council. The service provides social work services and arranges care for disabled adults. We aim to overcome barriers that prevent people from accessing mainstream society and support people to live independently and safely.
How we will work with you
How can I get support with a Brain Injury?
Our hospitals teams provide Social Work Care management to adults with Brain Injuries to anyone aged 18 years and older.
How can I get support for HIV/AIDS?
Our hospitals teams provide Social Work Care management to adults with HIV/AIDS to anyone aged 18 years and older.
How can I get support with a visual or hearing impairment?
Our Sensory Impairment Team provides specialist equipment, rehabilitation, support and care management to adults who have a visual or hearing impairment to anyone aged 18 years and older.
How can I get support from the Equipment & Adaptations Section?
The Equipment & Adaptations Section can assess your needs and make recommendations for you. They can make requests for equipment from the Sheffield Community Equipment Loan Service (SCELS). This service is for all age groups.
How can I get support to improve my independence, remain in my own home and access activities in the community?
The Community Support Service provides a range of meaningful community based daytime opportunities for disabled people. These are linked to leisure, education and employment. The service promotes social inclusion; provides social, emotional and functional rehabilitation, provides respite services and works with family carers. We also provide support with tenancies for people to remain in their own homes. This service is for anyone aged 18-65.
What is adult family placement?
A range of placements in the community are offered to all age groups through the Adult Family Placement Service. We will work to place Service Users in family settings for a variety of care needs, such as the provision of respite for a family/primary carer or to provide social contact to name two examples.
Who will assess my needs?
The following teams can discuss with you what sort of needs you have e.g. support with personal care, and will arrange for services to be provided that will meet those needs if you are eligible. They will regularly review whether or not your needs have changed and whether the plans that have been put in place are meeting your needs.
- Care Management Teams (North & South) Social Work Care management service to adults (18 to 64)
- Disability Access Team Access and Assessment Service to Adults (age 18-64)
- If you are 65 or over, you can find information about the support available from the Adult Access Team
