Our Services

Our Social care teams are based in locations around Sheffield. The teams are made up of social workers and care managers. Each team has a team manager. The staff in these teams are responsible for receiving referrals, carrying out assessments, arranging services and reviewing the support we provide to Older People and their carers in the city.

We also have hospital social care teams based at Sheffield’s NHS hospitals. Our hospital social care staff assess the needs of hospital patients, carers and relatives to make sure patients are discharged safely either back home or to somewhere else more appropriate to their needs.

Our intermediate care teams work with people who’ve had an operation, illness or fall. The teams can help people learn new and alternative ways of doing things and gain the confidence to return home.

We also have community mental health teams who provide assessment and care management services to older people who have a diagnosed mental health problem such as dementia or depression.
