Our Services for Older People
The Older People’s Assessment and Care Management service provides resources for the care and protection of vulnerable older people (65 years old and over) and their carers.
We make sure that
- your needs are assessed and met in an efficient and effective way
- you receive help to maximise your well-being and live independently with choice and control over your support
- you receive support to help you get back to living independently again if you have a crisis or your health gets worse.
We aim to provide care and support for older people in their own homes. Where this is not possible we may arrange care in a residential setting.
