Children with Disabilities Service
The Children with Disabilty service have now formed part of the Learning Disability and Difficulty Service. This means that education and social care will start to look more closely at how we can work together and deliver a better service for the young people and their families within Sheffield.
Within this it is our aim to support children and young people to live and grow within their own family and community, as long as it is safe to do so, and support them to make a valuable contribution to their communities.
What do we do?
This service offers social work support and guidance to children and young people who have a learning or physical impairment, and their families.
Will I get a service from you?
There are services which children, young people and parents can go to direct for help, advice and support, and there are other services that require an assessment of you and your child’s needs and circumstances, before a service may be offered.
What are the key services that result from an assessment?
Some families may be experiencing difficulties coping, and receive additional support. We will assess your needs and discuss with you and, with your agreement, with other professionals, to find a suitable service.
The services available are as follows:
Fieldwork Social Work
We are a small team specialising in delivering services to children with special needs/disabilities aged 5-16 years on a city wide service basis.
The criterion for access to this service is:
‘the child has a major physical impairment and/or moderate to severe learning disability, and there is an ongoing high level of dependency on others for personal care and the meeting of other basic needs’.
Transition Team
The team offer a transition planning service for children and young people leaving full-time education. We aim to support, with partner agencies school leavers with social, educational and employment opportunities. We ‘bridge the gap’ between Children’s and Adult Services.

Ryegate Centre
If you have a child under 5 years of age and have been to Ryegate Centre you might come into contact with the multidisiplinary social work team who are based there. The will be able to offer advice and guidance regarding benefits, early years support and, if required a social work assessment.

Children's Hospital
Social workers based here are part of the Children with Disabilities service. Within the hospital it forms part of a multi-disciplinary team and has close contact with medical and all other health staff. We work mainly with children with disabilities and chronic illness, and also have a role in ensuring the safety of children with injuries who are admitted to hospital.

Sargent Team
The team are based in the Haematology/Oncology clinic and in the main social work department in the Childrens’ Hospital. Our task is to provide practical, emotional and financial support to families where a child/young person (aged 0-24) has been diagnosed with a form of cancer. We work within the multidisciplinary team at the Children’s Hospital, on the TCT Unit at Weston Park and the Haematology Ward at the Hallamshire Hospital. We also have a welfare benefits advisor based with us in the Children’s Hospital for one day per week funded by the charity.

Respite Services
to give a break to carers, and offer support to children and young
people provided within a residential setting. We have two residential respite homes for children with severe learning difficulties, Mulberry Lodge and Rushey Meadow, and one home, Chancet Wood for children who have a physical disability, or are severely multiply disabled. Social workers assess children and families needs and send referrals to a Multi-Agency Panel, who make decisions as to whether an overnight service is appropriate, and which home is the most suitable for each child. Alternative services will be explored with families if an overnight service is not offered.

Direct Payments
Some families choose to consider arranging their own care via a Direct Payment rather than a directly provided service, see the Direct Payments website for more information.
MILAAP Service
Provides support and guidance to Asian women to increase their understanding of disability. It gives space, and a time to meet for mutual support and development.

How do I make a referral for assessment?
Families can make a referral by contacting First Point service available in Howden House. This can be done in person or by telephoning 0114 2734855.
If you are a professional wanting to refer a family, you can do this by initially contacting the Interviewing Officer, Ros Bishop on 0114 2736944. A Common Assessment Form will need to be completed to progress the referral.
