Help Leaving Hospital
What help is available?
When you leave hospital, you may be able to return home or you may need to move somewhere else. If you are about to leave hospital and may need an assessment or services from Social Services to make sure that you are discharged safely, staff on the hospital ward will refer you to the Hospital Social Work Service.
If you are a hospital patient, carer or relative, you can ask the ward staff to refer you to the Hospital Social Work Service. We will carry out an assessment of your needs.

You should contact the ward staff at the hospital where you or the person you are supporting is a patient.
