Care4you Home Support
Our Home Support service is provided as part of the overall Care4you service.
We are one of many organisations that provide care services in Sheffield and are registered and inspected by the Care Quality Commission (CQC) to ensure a quality service is provided.
Main areas of support
Our Home Support service concentrates on providing a specialist service in four main areas:
General long term home support is solely provided by a range of independent care providers, through contracts with the council.
Short Term Intervention
This team help people to regain their abilities or confidence in maintaining their independence, so they can continue to live in their own home.
The team provides support for a limited period of time. They aim to help people who may have had a stay in hospital, or who need help to avoid going into hospital or in other circumstances where someone needs help to regain their independence.
Mental Health
Staff in this team provide support to both adults who have a diagnosis of chronic and enduring mental health issues and their carers.
Adults with Complex Needs
This team provides support for adults who, for example, may be vulnerable, at risk of being abused, or have complex health and care needs, such as people who have been diagnosed with a long term health condition or who have extensive care needs that involve other professionals.
Night Care Visiting Home Support service
This service is for people who require support at home during the night. Carers are available to provide personal care, emotional support and encouragement. The service also supports customers who are receiving our Short Term Intervention services.
The service is available between 10pm and 7am, 365 days per year.
Night Care visiting is provided subject to an assessment of your needs.
What support is provided?
Help for people who have specific care needs, so that they can continue to live in their own home and be as independent as possible.
Home Support staff offer:
- personal care such as washing and dressing
- emotional support and encouragement
- assistance with medication
- help preparing meals
- help with shopping and other household tasks, if these are assessed as being needed as part of a care package.
The type and amount of care support provided to anyone will be individually tailored to that person’s needs. This package of care will be arranged through our Assessment and Care Management team.
A culturally appropriate service is available for all of Sheffield's ethnic minority groups.
Who do I need to contact?
We provide home support services subject to an assessment of your needs. The service will be provided by carers from Care4you or a privately run agency.
Will I need to pay for home support?
There is a standard charge for the service, although many people on income support receive a free service. We offer all home care clients benefits advice, to make sure they receive all the benefits they should be getting.
