The Community Assessment and Rehabilitation Team help you to...
 
  • stay at home, even when you’re not well.
  • return home from hospital as quickly as possible.
 
We can provide short term care quickly — either in your own home or at a resource centre, care home and so on.

Most people we help are 65 years old or over.  You might...
 
  • be recovering from a fall, and at risk of falling again
  • have a serious infection, such as a chest or urine infection
  • be dehydrated— if you’re not drinking enough fluids
  • be finding it difficult to get about
  • be recovering from injury after a fall.
 
We can help with support for a few days, or for up to six weeks.  We’re here to help you to be more independent and to improve your quality of life.
