How do I apply?
You can buy your card at any sports centre, swimming pool, Meersbrook Park (Parks and Countryside Head Office) or the Council 'First Point' enquiry offices.
Once you have completed your form, you will need to:
- provide evidence that you qualify - either by showing the benefit you receive on your bank statement or a letter from the provider, dated not more than 4 weeks
- show evidence that you are a Sheffield resident - a utilities bill etc, showing your name and address
- provide 2 photographs - family photos will do with your face cut out
- pay the cost of £3.00 for the card
Once you have satisfied the criteria and paid for the card you will recieve a voucher/receipt to use in the period up to your card being received. Your card should take no longer than 2 weeks to be processed. The card will last for 12 months and is not transferrable.
A parent/guardian must make the application on behalf of their dependent child.
Please note: Postal applications are not accepted.
