PIN Request Form

A Personal Identification Number (PIN) is required to request an item or view the ‘Your Membership’ section of the Library Catalogue.
 
A PIN gives the user the security of knowing that no other member of the public can access their personal details or request items in their name.
 
Your PIN can be requested in person from any public library in Sheffield, by telephoning 0114 273 4727 or by filling in our PIN request form below. NOTE: Please provide us with a contact telephone number if you use our online form, so we can ring and give you details of your PIN number, otherwise due to security reasons your PIN number will be posted out to you.
  
For security reasons staff may ask you to provide the following information:
  • Library card number or Smartcard number
  • Your name
  • The first line of your address
  • Date of birth
 
Your PIN can be changed in the 'Your Membership' area of the Library Catalogue.
 
 
Please note that if you have not used the library in the last 2 years your membership may have been cancelled.  This is in compliance with the Data Protection Act.  Please see our Joining the Library pages for more information.
