Housing Support for People with a Health or Disability Issue

Sheffield City Council's Housing Solutions Team can provide support if your current housing is unsuitable and your independence or health will be adversely affected without an urgent solution.
 
Examples of when we can help include
 
  • when you have a mobility, long-term health need or learning disability and a specific type of housing is required
  • when you need to leave hospital or institutional care and have no suitable accommodation to return to
  • when you suffer from mental ill health and your housing situation is having a detrimental effect on your ability to live independently.
  • when you need to live nearer your carer for essential support for daily living.
 
We are also able to provide information, advice and support to people with learning disabilities who wish to live independently or to families wanting to support their sons and daughters to live more independently in the community.

Assessment of Need

 
Where practical, most assessments will be completed in the your own home. Other assessments may be carried out over the phone, in hospital, at another agreed address. An assessment form will be completed by a Housing Solutions Officer and agreed and signed by you.
 
Where it is determined that re-housing is necessary to bring the outcome required to achieve health and well being then all achievable options are discussed to allow choices. This may include exploring all tenures, areas and types of property to meet a persons needs. All options will be discussed and if you wish to be re-housed, then an assessment may be made for urgent and immediate priority re-housing.
 
A medical condition alone does not mean there is a need for re-housing as will help you to address barriers to enable you stay at home where this is possible. If an assessment is made for urgent and immediate priority re-housing: you will receive a decision letter from the Housing Solutions Officer. If a priority is not awarded then you have the right of review.
 
Where priority for rehousing is awarded this priority will only last for 13 weeks. During that 13 weeks you will be expected to bid for suitable properties under the Council’s lettings system called ‘Choice Based lettings’. We will offer you help to bid for properties.
