- Completed and signed claim form
- Evidence of identity
- Evidence of address
- Evidence of National Insurance number
- Evidence of capital, savings and investments
- Evidence of earnings
- Evidence of other income
- Evidence of benefits, allowances and pensions
- Evidence of private tenancy and rent
- Evidence of other money paid out
Remember…
- To submit your claim form even if you are not able to supply all the supporting evidence
- To ask us for more time, if you are not able to supply the evidence straightaway
- Not to send valuable items through the post
- To provide original documents
- If you take your proofs to one of our offices, check the photocopies and get a receipt
- Contact us as soon as possible if you need any help
